These are the steps to be followed to add checkboxes on the final page after filling out an online admission form http://www.preadmit.com/Patient/<your hospital’s unique ID>

1. Login to http://www.preadmit.com.au/Hospital/<your hospital’s unique ID>
2. Click on Settings

3. Click on Final Page

4. You can add any tick box you would like to add on the final page, just click on Add Mandatory Checkbox

5. Fill out the required fields. Tick the Mandatory checkbox if you want to make it mandatory.

6. Make sure the Active tick box is checked.

7. Click Save
